Bromocriptine mesylate
Indications/Uses
Galactorrhea w/ or w/o amenorrhea due to hyperprolactinemia. Prolactin-dependent menstrual disorders & infertility (eg, secondary amenorrhea, ovulatory insufficiency & short luteal phase) & male hypogonadism. Inoperable prolactin-secretin adenomas or prior to surgical removal & as an alternative to surgery in patients w/ microadenomas. Adjunct to surgery or RT in acromegaly (can be used as monotherapy in special cases) & to l-dopa in symptomatic management of selected patients w/ Parkinson's disease who experience prominent dyskinesia or wearing off reactions on long-term l-dopa therapy.
Dosage/Direction for Use
Max: 20 mg daily. Galactorrhea w/ or w/o amenorrhea due to hyperprolactinemia & prolactin-dependent menstrual disorders, infertility & male hypogonadism Initial dose for establishing tolerance: 1.25-2.5 mg at bedtime. Gradually increase dose after 2-3 days to 2.5 mg bd or tds. Prolactin-secreting adenomas 1.25 mg bd-tds. Maintenance: 5-7.5 mg daily. Gradually increase dose over a period of several wk to 10-20 mg daily. Acromegaly Initial dose for establishing tolerance: 1.25-2.5 mg at bedtime. Gradually increase dose over a period of 2-4 wk to 10-20 mg daily. Parkinson's disease Initially 1.25 mg at bedtime. Maintenance: 1.25 mg bd; may gradually increase by adding 2.5 mg daily in divided doses once every 2-4 wk.
Administration
Should be taken with food.
Contraindications
Sensitivity to ergot alkaloids. Uncontrolled HTN & toxemia of pregnancy.
Special Precautions
May effectively lower prolactin levels in patients w/ pituitary tumors but does not obviate the necessity for RT or surgery. Possible pulmonary infiltrates, pleural effusion & thickening in long-term treatment. Immediately w/draw medication if fibrotic changes in retroperitoneum are diagnosed & suspected. Hypotension. Periodic BP monitoring during the 1st few days of therapy. Immediately discontinue use & promptly evaluate patient if severe progressive or unremitting headache, other CNS toxicity signs or HTN develop. Concomitant use w/ phenothiazines or other medications known to lower BP. Avoid alcohol. Concomitant use w/ other ergot derivatives & any potential vasoconstrictor drug is not recommended. Acromegalic patients w/ history or evidence of peptic ulceration. Possible recurrence in long-term continuous dosage in amenorrhea/galactorrhea. High doses in Parkinson's disease may be associated w/ mental confusion & disturbances. Patients w/ history of MI; residual, atrial, nodal or ventricular arrhythmia. Periodic evaluation of hepatic, hematopoietic, CV & renal function in patients receiving antihypertensive medication. Concomitant use w/ erythromycin; domperidone. Severe renal or hepatic disease. Dizziness may occur; caution against activities requiring rapid & precise responses eg, driving an automobile or operating dangerous machinery. Women of childbearing potential should use a reliable contraception method; conduct pregnancy test at least every 4 wk during amenorrheic period & every time menstrual period is missed once menses are reinitiated. Pregnancy.
Adverse Reactions
Nausea, vomiting, headache & GI side effects (eg, abdominal pain, diarrhea & constipation).
Drug Interactions
Increased severity & incidence of AR w/ alcohol. Concomitant use w/ other ergot derivatives & any potential vasoconstrictor drug is not recommended. May increase plasma levels w/ erythromycin. Possible increased serum prolactin & partially blocked anti-tumourigenic effects w/ domperidone in patients w/ prolactinomas.
